A funny thing happened on the way to reving for 3.4...  I decided I didn't
like the organization of this document.  Enter snowball effect.
Rewrite overview into a true overview.  Better discussion of boot vs.
install media.  Explainations of options, etc.  Lay groundwork for more
platform support.  Renumber document (*sigh*...3.4 will bring another
partial renumber). Sections 4.1 through 4.4 have been substantially
reworked.

Sync TOC with headers, standardize header layout after discussion with
Tom Cosgrove and JMC@ (first word capitalized, no trailing period.  YES,
this is debatable, no "perfect" answer, but one answer beats the random
way it was).  Lots of constructive input from Tom Cosgrove, thanks!

Note: this is *NOT* the 3.4 document.
