INDEX format change

Folks with scripts that extract information from ${PORTSDIR}/INDEX
should take note that the format has slightly changed.  A new field
for the library dependencies has been inserted.  The pipe-separated
fields are now:

 1. distribution name
 2. port path
 3. installation prefix
 4. comment
 5. description file
 6. maintainer
 7. categories
 8. lib depends
 9. build depends
10. run depends
11. architectures
12. package cdrom
13. package ftp
14. distfiles cdrom
15. distfiles ftp

Also, all dependency entries are now taken verbatim from *_DEPENDS.
