Thank you! You're right, that's it. After setting pcib-probe-list to "1,2,3"
second network interface has appeared and works:

I'm still curious (1) why there are 2 PHYs on 1st controller, and
(2) why MAC addresses are the same. But these questions are solely come
from curiosity (ifconfig hme1 lladdr other-MAC works properly).
